Penalties for Failure to Provide Information on a Prescribed Form
You are liable to a $100 penalty if you omit information on a prescribed form. In the case of form TP-1086.R.23.12-V, Costs Incurred for Work on an Immovable, the penalty is $200 or more.
However, you will not be charged a penalty for omitting information about someone else's identity (name, date of birth, address, occupation, social insurance number, Québec enterprise number, QST registration number, etc.) that you were unable to obtain despite your best efforts. In such a case, the other person is liable to a penalty for not giving you the information.
