Complete form CO-737.18.17.FS for any corporation or partnership that, for a taxation year or fiscal period, may be eligible to claim the credit for employer contributions to the health services fund in respect of eligible activities relating to one or more large investment projects.
Do not complete form CO-737.18.17.FS if the corporation or partnership made an irrevocable election with the Minister of Finance to use the alternative calculation method for a large investment project it is carrying out.
