Amending a GST/HST or QST Return

If you want to amend a GST/HST or QST return, you must complete and send us one of the following forms:

FPZ-2500-V, Request to Amend a Return, if the return did not include any schedules
FP-2500.E-V, Request to Amend a GST/HST and QST Return Filed Online, if the return included a schedule

You must complete a separate form for each period for which you are requesting a change.
