- you are a mandatary or a person authorized by a trustee that has signed a Québec education savings incentive (QESI) agreement with Revenu Québec;
- your business would like to register as a new registered education savings plan (RESP) trustee or promoter that wishes to offer the QESI to its clients.

Any request emailed to us that does not include a duly completed copy of form SW-172-V will be rejected.
