Complete this form if you are a First Nations member (Indian registered under the Indian Act) or a band council and you want to claim a refund of the tobacco tax you paid on tobacco products purchased for your own consumption at an establishment that a retail vendor operates on a reserve or in a First Nations settlement in Québec.
