RL-5 Slip: Benefits and Indemnities (courtesy translation)

The RL-5 slip must be filed by organizations such as the Ministère de l'Emploi et de la Solidarité sociale, the Commission des normes, de l'équité, de la santé et de la sécurité du travail (CNESST) and the Société de l'assurance automobile du Québec (SAAQ), to report the amounts of social assistance payments and indemnities paid to beneficiaries.
The information on the RL-5 slip is used by beneficiaries to complete the personal income tax return (TP-1-V).
