General GST/HST and QST Rebate Application

This form can be used to claim a rebate of the GST/HST and QST paid on certain property and services.

You can submit a copy of the form that you've completed onscreen and saved on your computer using the online service for submitting consumption tax rebate applications, which is available in My Account for individuals,  My Account for businesses, My Account for professional representatives or through clicSÉQUR express. It's quick, easy and environmentally friendly!

Instructions for rebate applications filed during the GST/HST break from December 14, 2024, to February 15, 2025

  1. Complete the lines and columns marked with a diamond, if applicable.  
  2. If you are applying online, include a scan of your original receipts with your GST rebate application and send everything through My Account.
  3. If you are applying by mail, enclose a paper copy of your receipts with your GST rebate application (these will not be returned to you) and mail everything to:

Follow the instructions below before clicking the Fillable PDF Document button at the bottom of the page.

  1. If you have already opened the fillable form, close it without saving it.

  2. Download Adobe Reader. If the software is already installed on your computer, go directly to step 4.

  3. After downloading and installing Adobe Reader, close it.

  4. Place your cursor on the Fillable PDF Document button at the bottom of the page and right click to display a menu.

  5. Select Save link as.

  6. Save the document to your desktop or the folder of your choice. Do not open the document in the download screen that appears.

  7. Once the document is saved, right click on it to display a menu.

  8. Click Open with and select Adobe Reader.

  9. Complete and save the PDF.

Incompatible browsers

Adobe Reader must be installed because some browsers (e.g. Google Chrome, Safari and Microsoft Edge) do not allow PDFs to be completed onscreen. If you do not want to install Adobe Reader, click the PDF Document button, print the form, complete it by hand and mail it to us.
