Credit for Contributions to the Health Services Fund for a Large Investment Project – Alternative Calculation Method

Complete form CO-737.18.17.FH for any corporation or partnership that is claiming the credit for contributions to the health services fund as an employer for one or more large investment projects for which it has made an irrevocable election with the Minister of Finance to use the alternative calculation method.
A corporation must use form CO-737.18.17.FH for the first taxation year covered by the election and subsequent taxation years. A partnership must use it for the first fiscal period covered by the election and subsequent fiscal periods.
