Application for a Fuel Tax Refund and a QST Rebate for Public Carriers

This form must be completed by a public carrier in order to claim a refund of the fuel tax paid on fuel used for public transport and, if applicable, a rebate of the Québec sales tax paid on the fuel used that gives entitlement to the fuel tax refund.
