Joint Designation of an Amount Respecting the PRPP/VRSP of a Deceased Member

This form is for the legal representative of a member of a pooled registered pension plan (PRPP) (including a voluntary retirement savings plan [VRSP]) who received an amount under the plan further to the member's death and a qualifying survivor of the member who is entitled to all or part of the amount in full or partial satisfaction of his or her rights as a beneficiary of the deceased's succession. By completing the form, the representative and the qualifying survivor can jointly designate the amount received by the legal representative under the plan as having been paid to the qualifying survivor and not to the legal representative.
