This form cannot be used to revoke an authorization or power of attorney.

To revoke an authorization or power of attorney granted to a designated person, you must:

use the online service in My Account for revoking an authorization or a power of attorney; or
complete form MR-69.R-V, Revocation of Authorization to Communicate Information or of Power of Attorney.
