Special Tax Relating to the Tax Credit for Cultural Patronage

This form is for individuals (other than a trust) who received the tax credit for cultural patronage pursuant to a pledge registered with the Minister of Culture and Communications and who, for the first time, have not paid the annual amount they committed to pay the qualified donee pursuant to the pledge. The form is used to calculate the special tax they must pay for any prescribed taxation year for not honouring the pledge.
