This form may be used to appoint a person or business to represent you with Revenu Québec for the purposes of your advance payments of the tax credit for home-support services for seniors.

Unlike form MR-69-V and the online service in My Account for individuals for granting a power of attorney, form MR-69.MD-V can be used to request that correspondence about your advance payments of the tax credit be sent directly to the person you appointed to represent you.
