• the first and last names of the individual
• the individual's identification number or the last three digits of his or her social insurance number
• the individual's professional representative number, where an individual in business has been accredited as a professional representative
• the individual's full address

To grant an authorization or a power of attorney to a business, you must provide:

• the business name
• the business's identification number or Québec enterprise number (NEQ)
• the business's professional representative number, if applicable
• the business's full address
