If you received an identification label, place it in the appropriate space on your return—even if some of the information on the label is incorrect. If the name is incorrect, complete lines 1 and 2 and contact us (see below). If the name is correct but you want to make a change, contact us as soon as possible.

If the address is incorrect, complete lines 7 through 9.

Change of address after you file your return

If your address changes after you file your return, contact us as soon as possible. For more information, see Change of Address.
