This form should be completed by any person who wishes to contest in the Small Claims Division of the Court of Québec any assessment, any allocation made under section 31 of the Tax administration Act, any determination of a property tax refund or any decision handed down under section 65 of the Act respecting the Québec Pension Plan. The Respondent in such an appeal is the Agence du revenu du Québec.
