How to Report a Failure to Meet Fiscal Obligations

Based on the information provided to us, we will determine whether or not we can take steps against the individual or business. For this reason, it is important that you send us all relevant information at your disposal.

The information and documents listed below are useful to us when processing a report:

  • the type of fiscal obligation not being respected
  • the name and contact information of the business concerned
  • the name and contact information of the individual concerned
  • the period during which the events took place
  • relevant information (the amounts involved, the frequency with which the events took place, the names of the persons involved, a description of the premises, any relevant addresses, the field of activity, the Québec enterprise number (NEQ) of the business concerned, etc.)
  • documents in your possession, or that you know exist, such as contracts, correspondence and bills (if you do not send us the documents, provide us with information about where they are stored)
  • any other information deemed relevant

You can make a report in one of the following two ways:

  • call us, toll-free, at 1 855 208-1131
  • complete a copy of form LM-6-V, Report of Failure to Meet Fiscal Obligations, and send it to us at one of the addresses listed below

Once a file has been opened regarding a reported failure to meet fiscal obligations, all the information contained in that file is confidential and protected under the Act respecting Access to documents held by public bodies and the Protection of personal information. We therefore cannot inform you of any action we take or of the results of our analysis.
